Online Annual Registration

Please review all demographic information to ensure we have the most up-to-date information. 

If you find an error, please contact our Central Enrollment office at 248-675-3400 and they will help guide you in making a correction.  

Please review and update all student contact information. Novi Community School District would like two additional contacts (in addition to parents) to reach out to in the event of an emergency, or someone who, in the event you are unable to do so, could pick up your child.  If a contact requires a change of contact information, please select the pencil icon to the right of the contact. 

Please note the following regarding parent contact information changes.  

·  Duplicate parent contacts – Please do not delete a duplicate parent contact.  Please contact parentportal@novik12.org for resolution. 

·  Do not delete or add a parent/guardian. If changes need to be made, please contact your child's school. 

·  If you do not have consent to alter a parent or guardian’s contact information (address, phone, etc.), please do not make any changes to the other parent or guardian’s contact information.  

·  If there has been a custody change that affects how the school interacts with either parent in relation to the student, please contact your child’s school and they will assist you.  The school office must have the custody agreement or court order on file.  

·  School Pickup – Please check this box if the Emergency Contact is authorized to pick up your child.   

Adding an Additional Emergency Contact

If you wish to add a contact to your child’s profile, select Add to the right just above the first contact.  Please enter their first and last name, relationship to the child, address, phone number and email address.  

Deleting a Contact

To delete a contact, please select the X to the right of the contact. 

Empty Forms-This form is designed to clear upon submission and will appear Empty.  This is to be expected.  If you would like to confirm that your changes were received you may revisit the form and see if the changes are reflected. 

Please do not delete a mother, father or guardian from your student’s record.

Please complete the Residence Survey by noting your current living arrangements.  

Your previous selection will be highlighted.  If no change is necessary, simply sign the form by typing your name and check the box to consent to an electronic signature.

Please complete the Permissions/Agreements page.   Previous selections are reflected in the form. 

We understand that some of this information has already been collected at the school level. For consistency and accuracy, we need you to update this information here as well. We apologize for any inconvenience. 

This page will look differently for students at various levels (e.g., Elementary, Middle School or High School). 

These permissions will be updated yearly through Online Annual Registration. 

Please acknowledge the form by making a selection on the page. 

Please complete the Health Information.  

If your child requires one of the various health plans, they can all be found by following the link in the form, at the end of the registration module or by visiting our website.  

If you already have your health plan completed and signed by your physician, you may upload it directly into the Parent Portal using the Student Health Form Upload form which you can find under Forms = General.

Please remember to have your child's physician authorize the health plan and return it along with medication to your child's building.  

Use this form to select transportation for your child.  

Our technology agreements have been updated.  All students need to accept the Acceptable Use Agreement.  

If you are in possession of a NCSD issued device, you must also accept the Student Computer Use Agreement.

You will find a link to purchase insurance, if you wish, to protect you against loss, theft, accidental breakage, etc.

Please acknowledge that you have reviewed and understand the Extended Absences Policy.  If you have any questions, please reach out to your child's school.
